Private Healthcare Statement
At Strathesk Medical Practice we recognise that with increasing demands on the NHS, and as waiting times for NHS secondary care services have increased, some patients may choose to access the Private Medical Sector.
Under the NHS General Medical Services (GMS) Regulations, essential services are defined as those delivered at the discretion of the GP, in consultation with the patient. This means that a GP will only carry out investigations or prescribe medication when it is necessary for the care they are providing and where they are the responsible clinician. Please be aware that NHS GPs are not obligated to support or manage aspects of private treatment that fall outside their NHS responsibilities. If you choose to pursue care through the private sector, any follow-up or ongoing treatment related to that care may need to be managed by your private provider. If you are unable to obtain follow up treatment for reasons such as the provider being overseas, then there are private hospitals and GP services who will be able to assist you. It is important that you investigate your options and costings before making your decision to go private for surgery / investigations.
BMA guidance states that if general practices receive requests from private providers to arrange tests or investigations, it is important to note that complying with such requests - regardless of the GP’s management and treatment of the patient - is outside the scope of NHS primary medical services.
Clinics and Services
C Card Service
Our practice is a C Card point, which can be used by anyone aged over sixteen - you do not need to be registered at our practice.
Simply show your C-Card Direct at reception. If you do not have a card we can supply one for you.
This service is available Monday to Friday, 9:30am to 4:30pm.
Visit the C Card website for information on other free contraception services available
Cervical Smears
The Cervical Screening test (smear test) is designed to check your cervix (neck of the womb) for the Human Papilloma Virus (HPV). HPV causes 99% of all cervical cancers. Cervical screening can stop cervical cancer before it starts.
If you have had HPV vaccinations, this will prevent around 75% of cervical cancer cases, but screening is still needed to pick up any other cervical cell changes.
You will receive a letter from the Health Board to let you know when your next smear appointment is due. When you receive your letter, please telephone the practice to book an appointment with one of our Practice Nurses.
If you are eligible for cervical screening (aged 25 to 64 years old and not had an appointment for over five years) but have not received a letter, then telephone us anyway.
It may take up to 10 weeks for you to get your results by letter. The results will also be sent to the person who took your test and your GP.
If you would like more information on cervical screening then please visit the NHS Lothian Cervical Screening Website.
Mental Health
Please note that our mental health nurse cannot prescribe medication or issue a fitnote.
We have our own primary care mental health nurse that works within the practice.
If you are aged between 18 to 65 years old and need help with your mental health or wellbeing, you can call the practice on 0131 322 9333 to book an appointment.
There is also a wide range of local services that can help you which can be found on our mental health page.
Non NHS Private Work
See our Non NHS Private Work page for details about the private services we offer.
Physiotherapy
Every GP practice in Midlothian has an Advanced Physiotherapy Practitioner who works within the practice who you can speak to if you have a muscle, bone or joint problem.
They are all highly experienced and can assess and diagnose any musculoskeletal conditions, provide you with an explanation of your condition, appropriate exercises and discuss management options. This may include referral to your local Physiotherapy department for ongoing physiotherapy, pain management advice, X-ray or referral for a specialist opinion.
See our appointments page for details
Low Back Pain
If you have pain in your lower back (including sciatica) you can now self-refer directly to a new digital support service which offers same-day appointments 24/7. The service is delivered by NHS Lothian’s clinical partner, Flok Health.
Private Shared Care Agreements
We recognise that with increasing demands on the NHS, some patients may choose to access the Private Medical Sector for diagnosis or management of health conditions, including ADHD and Gender Dysphoria.
This can lead to requests for NHS prescriptions of medications initiated privately but that require specialist monitoring—often referred to as “shared care.” As a Practice, we have agreed that any such request from the Private Sector will be assessed on a case-by-case basis using set criteria. While some practices in Scotland decline all private shared care requests, in line with BMA guidance, we assess each one individually.
NHS Lothian has robust processes in place for Shared Care Agreements between Primary and Secondary care. These standards may also be applied to Private Shared Care requests but we cannot be certain. Examples include the private provider being responsible for ongoing monitoring such as blood tests, biometrics (e.g. weight, height, blood pressure), and regular review, as well as handling of test results.
Where Shared Care is agreed, failure to maintain appropriate monitoring with the private provider or lack of compliance with follow-up protocols will result in the termination of the agreement and cessation of NHS prescribing. It is essential to understand that ongoing NHS prescriptions are not guaranteed for medication started in the private sector.
If we are unable to accept a Private Shared Care request, we will be happy to discuss an NHS referral in a routine appointment—either for a second opinion or for consideration of transfer of care. Please note, however, that we cannot prescribe or monitor treatment in the interim period, and any ongoing private care costs would remain your responsibility.
We understand how frustrating and disappointing this situation may be for some individuals. However, it is important to be aware of the clinical and financial implications of pursuing private treatment, including the need for consistent engagement with private services.
